Step 1: Save the Dates

CHW Virtual Meeting Dates by Cohorts

  • Must be at least 18 years old.
  • Must have at least a high school diploma or GED.
  • Must have a personal email address (a computer and internet access is required). Request support for unmet requirements (including computers, computer training, and access to the Internet).
  • All these types of CHWs carry out one or more functions related to health care delivery and are trained in some way for the interventions they are expected to perform

Step 2: Program Requirements

This CHW program expects the individual to read, understand, and agree to the following section:

2B) CHW Pre Assessment

2C) Sexual Harrassment Training

2D) Resume Upload

2E) Academic & Photo Share

2F) Buddy System

Step 3: Financial Documents

This program provides you with a stipend as a form of social support. We require the following documents to establish your eligibility for the Department of Labor:
